2 – REGISTERING VIA INTERNET

Save time by registering on our website with your login in the access reserved for registrants, "Registration" menu, you have access to your registration form.

You can:

• Consult your old orders (validated orders)
• Create or modify an order (orders awaiting validation), save your order in a "basket" before validating it definitively (several baskets possible at the same time).

3 – REGISTERING VIA POSTAL MAIL

Carefully fill out the registration form sent to you by postal mail:

• enter all your contact details (postal address, email, etc.),
• for each programme selected, specify the desired amount in the appropriate box,
• Enter the total amount for your order on page 1 and add shipping costs if necessary,
• date, sign and send back the ORIGINAL form, with ALL its pages, by postal mail ONLY.
• include your purchase order or payment.

Your registration form will be scanned, so please take care to fill it out clearly and to return ALL its pages.

4 - IMPORTANT

Whichever registration method you use, please keep in mind that:

• We strive to produce an adequate number of vials of the control preparations. Nevertheless, the number of vials is limited. We therefore advise you to order your programmes as soon as possible.
• Registrations are confirmed in order of receipt and depending on available stocks.
• Orders will only be confirmed once all outstanding invoices have been settled.
• Orders are confirmed by e-mail as soon as ProBioQual validates them. It is up to you to check that the validated order matches your request.

Priority is given to complete registration applications by order of receipt.

The registration form, our brochure and our general terms and conditions of sale form the basis of the contract.
